The mortality of elective surgical treatment of abdominal aortic aneurysms has decreased over the last decades, but the mortality observed after surgical treatment of ruptured abdominal aorta aneurysms does not seem to decrease significantly. Open surgery was the primary treatment, but the introduction of endovascular techniques has changed the algorithm for treatment of ruptured aneurysms in major medical centers. The overall mortality of abdominal aortic ruptured aneurysms still remains high, despite significant improvements in surgery, anesthesia, and postoperative management in intensive care. This led surgeons to select patients according to their comorbidities and preoperative prognostic factors, in order to identify patients with a hope of surgical success. We present a review of the current management of ruptured infra renal abdominal aortic aneurysms.
